Where would I find official instructions for updating my complete Unity 3.X installation (Engine, Editor, Web Player)?
I find responses to quite a few problems that people have had, no but instructions as to how to potentially avoid update problems in the first place.

There aren't any, aside from "run the installer". Not sure what you mean by "Engine, Editor" since those are the same thing. I would recommend keeping the old version of Unity just in case, and of course make sure you have backups of your projects (which you should have anyway).
Up to 2 attachments (including images) can be used with a maximum of 524.3 kB each and 1.0 MB total.
