There's a tutorial on design3 that walks through making high scores with Unity using WWW and PHP, but WWWForm isn't used. I would definitely check out the documentation kevork suggested as well.

Up to 2 attachments (including images) can be used with a maximum of 524.3 kB each and 1.0 MB total.
